L&#39;&eacute;tude d&eacute;montre que l&#39;ajout de l&#39;endomicroscopie avec Cellvizio comme adjuvant pour guider les biopsies fournit un rendement diagnostique significativement plus &eacute;lev&eacute; pour la dysplasie et le cancer et r&eacute;duit l&#39;erreur d&#39;&eacute;chantillonnage par rapport aux biopsies al&eacute;atoires &agrave; quatre quadrants seuls, constituant la m&eacute;thode standard de diagnostic.<\/p>\n<p>Les m&eacute;thodes de d&eacute;pistage et de surveillance endoscopiques existantes sont insuffisantes pour d&eacute;tecter l&#39;oesophage de Barrett ou le cancer de l&#39;oesophage, car l&#39;utilisation du protocole de Seattle, qui constitue la m&eacute;thode standard de diagnostic, est sujette &agrave; des erreurs d&#39;&eacute;chantillonnage et &agrave; une faible sensibilit&eacute; et sp&eacute;cificit&eacute;. Lors de la Digestive Disease Week<sup>&reg;<\/sup> (DDW) qui s&rsquo;est tenue au mois de mai 2022, le Dr Bashar Qumseya a pr&eacute;sent&eacute; des donn&eacute;es<sup>2<\/sup> bas&eacute;es sur environ 5 millions de personnes et a d&eacute;montr&eacute; que le taux de cancer de l&#39;oesophage a presque doubl&eacute; entre 2012 et 2019 chez les personnes &acirc;g&eacute;es de 45 &agrave; 64 ans. En outre, la pr&eacute;valence de l&#39;oesophage de Barrett &#8211; le seul pr&eacute;curseur connu de l&rsquo;ad&eacute;nocarcinome de l&rsquo;oesophage (ACO) &#8211; a augment&eacute; d&#39;environ 50% dans ce groupe d&#39;&acirc;ge au cours de la m&ecirc;me p&eacute;riode.<\/p>\n<p>Dans la m&eacute;ta-analyse publi&eacute;e dans TIGE, les auteurs ont inclus 9 &eacute;tudes, pour un total de 688 patients et 1 299 l&eacute;sions, et ont &eacute;valu&eacute; les avantages de la pCLE en tant qu&#39;adjuvant aux biopsies al&eacute;atoires &agrave; 4 quadrants dans la surveillance des patients atteints de l&#39;oesophage de Barrett pour la d&eacute;tection pr&eacute;coce de la dysplasie et du cancer. La sensibilit&eacute;, la sp&eacute;cificit&eacute; ainsi que la valeur pr&eacute;dictive n&eacute;gative de la pCLE par patient &eacute;taient respectivement de 96%, 93% et 98%. Par rapport aux biopsies al&eacute;atoires, les augmentations des taux de d&eacute;tection absolu et relatif de n&eacute;oplasie par patient avec le pCLE &eacute;taient significatives et &eacute;gales &agrave; 5% et 243%, respectivement.<\/p>\n<p>Par ailleurs, lors du Congr&egrave;s mondial d&#39;endoscopie gastro-intestinale ENDO 2022, les r&eacute;sultats d&#39;une &eacute;tude r&eacute;trospective multicentrique des dossiers<sup>3<\/sup> de 60 patients atteints par l&#39;oesophage de Barrett et orient&eacute;s vers une surveillance ou un traitement endoscopique ont &eacute;t&eacute; pr&eacute;sent&eacute;s. Les auteurs ont examin&eacute; les diff&eacute;rences dans l&#39;utilisation des services de sant&eacute; en gastro-ent&eacute;rologie pour 8 &eacute;l&eacute;ments\/services parmi les patients&nbsp;imag&eacute;s par Cellvizio en tant qu&#39;adjuvant par rapport au diagnostic standard seul. La cohorte Cellvizio a obtenu des scores moindres de l&rsquo;ordre de : 1,04 endoscopie et service d&#39;anesth&eacute;sie de moins, 7,49 flacons de biopsie de moins, 1,30 ablation de moins, et 1,46 service de brosses cytologiques de moins.
