Son protocole a \u00e9t\u00e9 d\u00e9fini en amont par un comit\u00e9 d&#8217;\u00e9valuation d\u00e9di\u00e9, constitu\u00e9 par l&#8217;agence et r\u00e9unissant des anatomo-pathologistes, des gastro-ent\u00e9rologues, des m\u00e9thodologistes et des biostatisticiens. Les travaux ont suivi les m\u00e9thodologies Cochrane et PRISMA, avec \u00e9valuation de la qualit\u00e9 par l&#8217;outil QUADAS-2, mod\u00e9lisation bivari\u00e9e \u00e0 effets al\u00e9atoires des performances diagnostiques group\u00e9es, analyses de sensibilit\u00e9 pr\u00e9-sp\u00e9cifi\u00e9es et tests formels de biais de publication.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Les co-auteurs sont affili\u00e9s \u00e0 la NECA, \u00e0 la CHA University School of Medicine, \u00e0 l&#8217;Asan Medical Center (University of Ulsan College of Medicine), \u00e0 la Yonsei University College of Medicine et au Seoul St. Mary&#8217;s\u00a0Hospital (The Catholic University of Korea College of Medicine). Les auteurs d\u00e9clarent n&#8217;avoir aucun lien d&#8217;int\u00e9r\u00eat. Mauna Kea Technologies n&#8217;est intervenue ni dans la conception, ni dans la conduite, ni dans le financement, ni dans l&#8217;interpr\u00e9tation de cette revue.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>D\u00e9tection accrue des l\u00e9sions et r\u00e9duction du nombre de biopsies&nbsp;<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">En surveillance de l&#8217;oesophage de Barrett, l&#8217;ajout du pCLE \u00e0 l&#8217;endoscopie en lumi\u00e8re blanche haute d\u00e9finition a port\u00e9 le taux de d\u00e9tection de la dysplasie par patient \u00e0 28 % (14 patients sur 50) contre 10 % (5 patients sur 50) (p = 0,04). Chez les patients atteints d&#8217;un ad\u00e9nocarcinome gastrique peu coh\u00e9sif, les biopsies guid\u00e9es par pCLE ont mis en \u00e9vidence du tissu malin chez 65 % des patients (19 sur 29), contre 30 % (10 sur 32) avec des biopsies dirig\u00e9es par la seule lumi\u00e8re blanche (p = 0,01). Dans un essai randomis\u00e9 conduit chez des patients suspects de l\u00e9sions gastriques pr\u00e9canc\u00e9reuses, l&#8217;association du pCLE \u00e0 l&#8217;imagerie FICE a produit un rendement diagnostique par biopsie de 75,1 %, contre 31,5 % avec la FICE seule (p &lt; 0,001).&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Cette m\u00eame \u00e9tude randomis\u00e9e a montr\u00e9 que le ciblage guid\u00e9 par pCLE r\u00e9duisait de 48,5 % le nombre moyen de biopsies par patient, de 6,8 \u00e0 3,5 (p &lt; 0,001). Dans une cohorte s\u00e9lectionn\u00e9e de 13 patients pr\u00e9sentant un oesophage de Barrett avec des irr\u00e9gularit\u00e9s muqueuses discr\u00e8tes, sans l\u00e9sion macroscopique identifiable, l&#8217;ajout du pCLE a modifi\u00e9 la prise en charge th\u00e9rapeutique en temps r\u00e9el dans 69,2 % des cas (9 sur 13) : chez certains patients en conduisant \u00e0 une r\u00e9section endoscopique lorsqu&#8217;une l\u00e9sion de plus haut grade \u00e9tait suspect\u00e9e, chez d&#8217;autres en \u00e9vitant une r\u00e9section ou une ablation inutile lorsque la n\u00e9oplasie n&#8217;\u00e9tait pas confirm\u00e9e.\u00a0<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Performances diagnostiques&nbsp;<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Les auteurs concluent que le pCLE constitue un compl\u00e9ment s\u00fbr et fiable de l&#8217;endoscopie digestive haute standard, en particulier lorsque les l\u00e9sions sont peu visibles ou qu&#8217;une confirmation histologique est d\u00e9terminante. Dans les \u00e9tudes comparant directement les deux approches, l&#8217;ajout du pCLE a syst\u00e9matiquement montr\u00e9 une sensibilit\u00e9 sup\u00e9rieure \u00e0 celle de la lumi\u00e8re blanche seule, pour une sp\u00e9cificit\u00e9 comparable ou l\u00e9g\u00e8rement am\u00e9lior\u00e9e, ce qui conforte son r\u00f4le d&#8217;outil de caract\u00e9risation venant compl\u00e9ter une endoscopie conventionnelle de qualit\u00e9, et non s&#8217;y substituer.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Par rapport \u00e0 l&#8217;histopathologie, la sensibilit\u00e9 group\u00e9e s&#8217;\u00e9tablit \u00e0 89 % pour les n\u00e9oplasies oesophagiennes comme gastriques ; la sp\u00e9cificit\u00e9 group\u00e9e atteint 79 % pour les l\u00e9sions oesophagiennes et 95 % pour les l\u00e9sions gastriques. La discrimination s&#8217;est r\u00e9v\u00e9l\u00e9e solide et constante pour la dysplasie de haut grade et les l\u00e9sions franchement n\u00e9oplasiques, avec une sensibilit\u00e9 group\u00e9e de 88 % et une sp\u00e9cificit\u00e9 de 87 %. Les auteurs pr\u00e9cisent que ces estimations group\u00e9es, en particulier la sp\u00e9cificit\u00e9 \u00e9lev\u00e9e observ\u00e9e pour les l\u00e9sions gastriques, doivent \u00eatre interpr\u00e9t\u00e9es avec prudence compte tenu de possibles effets li\u00e9s aux \u00e9tudes de faible effectif, et que la base de preuves provient majoritairement de centres acad\u00e9miques \u00e0 fort volume d&#8217;activit\u00e9.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Statut r\u00e9glementaire et prise en charge&nbsp;<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">L&#8217;endomicroscopie confocale laser est d\u00e9j\u00e0 \u00e9tablie dans les indications couvertes par cette m\u00e9ta-analyse. En Cor\u00e9e du Sud, la technologie a fait l&#8217;objet d&#8217;une \u00e9valuation favorable de la NECA en 2018, et la prise en charge de la minisonde GastroFlex\u2122 UHD ainsi que de l&#8217;acte d&#8217;endomicroscopie confocale dans les indications oesophagiennes et gastriques a \u00e9t\u00e9 ratifi\u00e9e par le minist\u00e8re de la Sant\u00e9 et de la Protection sociale en f\u00e9vrier 2020. Aux \u00c9tats-Unis, l&#8217;endomicroscopie optique est r\u00e9alis\u00e9e et cot\u00e9e au titre de codes CPT<sup>\u00ae<\/sup> de cat\u00e9gorie I d\u00e9di\u00e9s, et les applications digestives hautes constituent le socle des revenus r\u00e9currents par acte de la Soci\u00e9t\u00e9.
